Reserves the right to inspect the premises in/on which the rescued Boxer is living. If the adoption fee is not paid, or if any of the other conditions of this placement are not met, Heart of Ohio Boxer Rescue reserves the right to reclaim the adopted boxer. By submitting this application, you agree that Heart of Ohio Boxer Rescue may follow-up and claim the adopted Boxer at any time.

By submitting this form, I give Heart of Ohio Boxer Rescue permission to contact the listed veterinarian and ask him/her questions about the quality of my pet care.


The fee we ask for our lovable boxers is to pay for any necessary veterinary treatment incurred for the adopted boxer. I understand that the adoption of this boxer depends on the honesty of these answers and the promise to maintain my boxer's health and well-being as an inside pet, and to welcome this boxer into my lisfe as a member of the family. I additionally understand that my acceptance of this boxer is on an "as is" basis. It is my understanding that every possible precaution has been taken to see that this dog is healthy and suitable as a household pet. Any condition of health or temperament that is not known by Heart of Ohio Boxer Rescue is not the responsibility of Heart of Ohio Boxer Rescue, and I hold the Heart of Ohio Boxer Rescue harmless. I hereby additionally understand that, if for some reason I must relinquish my adopted boxer, I must return it only to the Heart of Ohio Boxer Rescue. This is a legal requirement of this contract.
